Recognize your responsibilities in helping to prevent risks.
Identify how to ensure a culture of safety at HMC.
Recognize barriers to incident reporting and how to overcome them.
Distinguish between different types of incidents.
Identify key steps in the incident reporting process.
Recognize relevant information to include in an incident report in the Electronic Incident Reporting System (EIRS).

Identify the purpose of a risk register.
Recognize the importance of using a risk assessment process.
Recognize how to identify risks proactively.
Recognize key actions to take in the evaluation step of the risk assessment process.
Use the risk assessment matrix (RAM) to grade the severity of a given risk.
Recognize how to use failure modes and effects Analysis (FMEA) as a proactive tool to assess risks in a critical high risk area or process.

This course is presented in four modules:

1. Risk Management at HMC

Provides an overview of risk management at HMC – What is risk and how is it assessed? What is the purpose of risk management in healthcare? What are your responsibilities? and How can you ensure a culture of safety at HMC?

2. Managing Risk Proactively

Explains each step in the risk assessment process including the governance structure, the use of risk registers and identifying, evaluating, recording and reviewing risks.

3.Reporting Incidents at HMC

Focuses on reporting risks and incidents, including why reporting is important and how to overcome barriers, what should be reported, and how.

4. Using Risk Assessment Tools

Introduces three risk management tools - Risk Assessment Matrix, Root Cause Analysis, FMEA – and how these tools can be used to better understand risk at HMC.
